  You don't need to fill it in now, as prior to starting your 6 week programme you'll be asked to complete an electronic version.
  However if you know you do, or think you might need to get the OK from your GP, or maybe a consultant, please print and complete a copy of this form and ask them to sign and date it below your signature to confirm you're ok to get started with us.
